fa بررسی اثر حضور پزشک در خطای ثبت کدهای جراحی توسط دستیاران جراحی و اثرات مالی ناشی از آن در بیمارستان رسول اکرم(ص) Assessing the Effect of the Physician's Presence on the Error of Recording Surgical Codes by Surgical Assistants and the Resulting Financial Effects in Rasoul Akram Hospital مدیریت خدمات بهداشتی درمانی Hospital Managment پژوهشی اصيل Original Research <div style="text-align: justify;">زمینه و هدف: این مطالعه با هدف بررسی اثر حضور پزشک در خطای ثبت کدهای جراحی توسط دستیاران جراحی و اثرات مالی ناشی از آن در بیمارستان رسول اکرم انجام شد. روش بررسی: مطالعه ی حاضر از نوع توصیفی-تحلیلی و آینده نگر بوده و در بازه زمانی سه ماهه ی زمستان ۱۴۰۰ و از طریق جمع آوری داده ها در بیمارستان انجام شد. به جهت مقایسه ی کسورات با توجه به حضور یا عدم‌حضور پزشک، کارشناسی در اتاق عمل مستقر شد که اطلاعات را در فرم های مخصوص وارد کند و بدین صورت انواع کسورات برای هریک از وضعیت های مذکور مشخص شد. از آزمونT-Test  برای تجزیه و تحلیل بین دو گروه(حضور و عدم حضور پزشک) استفاده شد و داده ها با استفاده از نرم افزارهای اکسل و SPSS تجزیه و تحلیل گردید. یافته ها: تعداد ۳۰۱ پرونده در زمان حضور پزشک جراح و ۳۰۰ پرونده در زمان عدم‌حضور پزشک جراح بر اساس فرمول نمونه گیری کوکران بررسی گردید. تفاوت درصد کسورات پنهان در زمان حضور و عدم‌حضور پزشک جراح معنادار نبوده است(۰/۰۷۸P-value=). با وجود این، تفاوت درصد کسورات آشکار در زمان حضور و عدم‌حضور پزشک جراح معنادار بوده است(۰/۰۲۴P-value=). همچنین تفاوت هزینه ی کسورات آشکار در گروه های جراحی معنادار بوده است(۰/۰۰۱P-value<) اما تفاوت هزینه  های کسور پنهان در صورت وجود یا نبود پزشک، معنادار نبوده است(۰/۴۳۵P-value=). نتیجه گیری: حضور یا عدم‌حضور پزشک جراح در میزان خطا در ثبت کدهای جراحی توسط کمک جراح و در نتیجه در درصد و هزینه های کسورات اعمال جراحی و خصوصاً در کسورات آشکار حق العمل جراح اثر دارد. ازاینرو برنامه ریزی برای حضور پزشک در مواقعی که خطای ثبت کدها توسط کمک جراح منجر به کسورات آشکار می شود، می تواند به مستندسازی دقیقتر و در نتیجه کسورات کمتر ناشی از خطای ثبت کدها کمک کند.</div> <div style="text-align: justify;">Background and Aim: this study aims investigating the effect of the physician's presence on the error of recording surgical codes by surgical assistants and the resulting financial effects in Rasoul Akram hospital. Materials and Methods: The present study was a descriptive-analytical and prospective study that was performed by collecting data in the hospital and reviewing the documents from Jun to March 2022. Therefore, in order to compare the deductions according to the presence or absence of the doctor, an expert was stationed in the operating room to enter the information in special forms, and thus the types of deductions for each of the mentioned situations were identified. T-test was used for analysis between the two groups (presence and absence of physician) and data were analyzed using Excel and SPSS software. Results: Three hundred and one records were reviewed during the presence of the physician and 300 cases during the absence of the physician based on Cochran sampling formula. The percent difference between hidden deductions in the presence and absence of the physician was not significant (P-value=0.078). However, the difference between the obvious deductions in the presence and absence of the physicians was significant (P-value=0.024). The difference in obvious deduction costs was significant in the presence or absence of a physician (P-value<0.001). But, the difference in hidden fraction costs in the presence or absence of physicians, was not significant (P-value=0.435). Conclusion: The presence or absence of a surgeon has an effect on the amount of errors in the registration of surgical codes by the surgeon’s assistant and, as a result, this matter affects the percentage and costs of surgical deductions, especially in the obvious deductions.
